Consent Forms
Unfortunately, forms are part of going to the doctor. QueerDoc tries to minimize forms. We make health history forms optional. You do have to sign the ones that mean you legally consent to receiving care from QueerDoc and agree to our policies. We also have special forms for different types of gender affirming care. The last three forms are so your primary care provider can give information to us, so we can give information to them, and so you can communicate with us through regular email (THESE THREE ARE ALL OPTIONAL). The copies below are for you to review.
You can fill all of these out online in our Patient Portal and do not need to do them here. This just for you to review.
